Too often, the individuals we serve arrive with chronic conditions that have gone unmanaged, whether due to homelessness, incarceration, or systemic barriers to care. Within the first week of entry, we support residents in completing comprehensive health screenings, including HIV and STI testing, and connect them to primary care, dental services, and any needed specialists. Our staff coordinate transportation, appointment scheduling, and medication pickup to reduce logistical barriers. They also support residents in understanding treatment plans and advocating for themselves in clinical encounters.
Daily coaching is provided across a broad range of life skills, particularly those that may have been underdeveloped due to institutionalization or trauma, including cooking, laundry, cleaning routines, personal hygiene, budgeting, and time management. These services are woven into everyday interactions, with an approach that is patient, repetitive, and grounded in mutual respect. Our staff guide rather than instruct, modeling skills and encouraging autonomy.
Group-based wellness programming includes topics such as meal planning on a budget, mindfulness and self-regulation techniques, managing sleep hygiene, and setting achievable goals. We also offer peer-led outings and recreational activities that encourage residents to reconnect with the joys of daily life, build community with one another, and foster emotional resilience. These services ensure that wellness is defined holistically as the ability to thrive in community, meet one’s own needs, and take pride in one’s growth.
